Martin Luther Care Center offers short-term, long-term, and memory care. Our care center is designed and named after neighborhoods.

For patients who no longer need hospital care, but are not ready to go home, Martin  Luther Care Center’s Transitional Care Unit provides additional care and therapeutic services to complete their recovery.  Martin Luther Care Center’s Transitional Care Unit, also known as Bridgeway, offers patients a bridge between hospitalization and a return to independence.

Most commonly, Bridgeway’s patients are referred by an area hospital and approved for admission by their doctor.  Typically, people come to Brigeway after surgery, a serious fall, illness, or a stroke. Bridgeway assists patients to stabilize and manage wounds, nutrition, diabetes, or intravenous medication.

Goals for achieving independence are set upon admission to Bridgeway.  Our team of caring professionals includes rehabilitation therapists, doctors, nurses, nursing assistants, licensed social workers, clinical dieticians, nutrition and culinary staff, chaplains, and recreation specialists.  In addition, we can arrange visits for you with your specialty doctors or dialysis unit.

Many clinic systems send doctors and nurse practitioners to Martin Luther Care Center to see patients. Please inquire whether or not your clinic provides on-site visits at the facility. As an Ebenezer managed community, and part of the Fairview Health Services system, our transitional care unit offers patients access to the Fairview Partners program.  If you are a Fairview Clinics patient the Fairview Partners program brings doctors and nurse practitioners right to your bedside to take care of your primary health care needs. We are also pleased to announce that we are a preferred provider for Health Partners

An average stay in Bridgeway is usually about 14 to 21 days.  An individualized plan of care is developed for each patient and is followed throughout a patient’s stay. During their stay most patients have two daily rehabilitation sessions with each therapy: physical, occupational, and/or speech. Our nursing staff assists patients with medication administration and personal cares and help patients learn to manage medications for use at home.  Our health professionals keep patients and loved ones informed of progress and outcomes.

We offer comfortable rooms in a beautiful setting. Patient rooms have televisions and telephones. Satisfying meals can be enjoyed in our dining room. Every member of our caring staff is dedicated to making your stay a pleasant experience.

Bridgeway neighborhood is distinct from the other neighborhoods with its own private secured courtyard, cable television, telephones and many other amenities. In addition, Martin Luther Care Center has a beauty salon, chapel and spiritual care program, transportation services, and regularly scheduled activities.
